2. Description of Pet (Example: Short Haired Grey Cat, Green eyes, Crooked Tail)

3. Sex of Pet: Altered or Unaltered

4. Lost/Found Pet wearing a collar? With or without tags?

5. Name of Pet

6. Date Last Seen/Date Found

7. Vicinity Last Seen/Vicinity Found

8. Contact Info (Emails will be listed on line & phone numbers will be kept private at the shelter.

9. IMPORTANT: If you have a digital photo, please attach it to your email. Photos help tremendously.
